Incident kill-switch — bulk revoke (keys:admin)

Required scope: keys:admin. Revokes a named set of keys (keyIds, 1-100) or ALL active keys (all: true) for the company — exactly one of the two. Every revocation writes its own audit row (per-entity attribution). The calling admin key can revoke itself.

429 — RATE_LIMITED (per-key limit, reads and writes alike; fails closed, so a limiter outage also returns 429). A limiter-down 429 is deliberately indistinguishable from a quota 429. Carries Retry-After (seconds).
